Learning Objectives

 

Provide an opportunity for health affairs faculty and preceptors to reflect on education and next steps as it relates to population health.
Discuss 'outside the box' strategies for addressing collaborative, team-based care
Identify challenges/opportunities to improve patient care and population health, broadly.
