Best Practices in Transplant Pharmacy in NC – Pre-transplant Evaluation Phase

The current practice of transplant pharmacy in the pre-transplant phase is not well defined. The goal of this webinar is to review the available literature on practice standards, highlight requirements needed for transplant program accreditation, and provide insight from transplant pharmacists across the state for their current practice at individual institutions. This webinar is geared to help improve transplant pharmacy practice across the state of NC. Objectives: 1. Provide an overview of the current practice of the transplant pharmacist in evaluating patients in the pre-transplant setting 2. Discuss pre-transplant documentation and involvement during multidisciplinary evaluation conferences 3. Highlight unique roles of the pharmacist in the pre-transplant setting (ie. designing antibiotic plans, vaccination clinics, medication management/interventions, etc)
