2026 Community Practitioner Program (CPP) Annual Meeting

  • The North Carolina Medical Society Foundation Community Practitioner Program (CPP) is a statewide practice support initiative designed to help community-based clinicians and practices successfully navigate the transition from fee-for-service reimbursement to value-based care models. As payers increasingly adopt alternative payment arrangements that reward quality, outcomes, patient experience, and cost efficiency, clinicians must adapt workflows, care coordination strategies, and population health approaches to remain sustainable and deliver high-value care. The 2026 CPP Annual Meeting will convene physicians, advanced practice providers, and practice leaders from CPP-participating practices across North Carolina to build knowledge and skills necessary for thriving in value-based care environments. Educational sessions will focus on key competencies including value-based practice transformation, behavioral health integration, patient engagement strategies, and clinician well-being and suicide prevention. Participants will gain insight into current Medicaid clinical priorities and expectations from managed care organizations, including updates related to Carolina Complete Health Network and evolving care delivery requirements. The program will also provide practical guidance on aligning practice operations with North Carolina Department of Health and Human Services priorities and Advanced Medical Home (AMH) Tier 3 standards, which serve as a foundation for high-quality, person-centered care in North Carolina. Through presentations and facilitated discussions led by experts from NC AHEC, the North Carolina Medical Society Foundation, and academic partners including University of North Carolina at Greensboro, participants will explore practical tools and strategies to support care coordination, risk stratification, behavioral health integration, and patient engagement within community-based practices. Additionally, the program includes suicide prevention training tailored to health professionals. This session will increase participants’ ability to recognize warning signs of suicide, respond effectively to individuals in crisis, and connect patients to appropriate resources, thereby strengthening the capacity of clinical teams to address mental health and safety concerns in their patient populations. By the end of the meeting, participants will be better equipped to: • Understand the evolving policy and payer landscape shaping value-based care in North Carolina. • Identify strategies to strengthen practice readiness for value-based payment arrangements. • Apply practical approaches to integrating behavioral health and improving patient engagement in primary care settings. • Recognize and respond to suicide risk among patients and colleagues using evidence-based approaches. • Implement tools and resources that support high-quality, coordinated, and patient-centered care. This educational activity supports clinicians in developing the knowledge and practical skills necessary to improve patient outcomes, enhance care delivery, and sustain successful community-based practices in an increasingly value-driven health care environment.
