According to the National Kidney Foundation, over 80,000 people die annually due to Chronic Kidney Disease (CKD), making it the ninth leading cause of death in the U.S. considering that late referral has been associated with higher mortality, morbidity and costs, the need to advance the management of CKD at the onset of disease is essential. It is imperative that primary care physicians have a firm grasp on the complexity of renal disease and transplant diagnoses, management and referral requirements, and post-transplant care.

- Identify risk factors leading to chronic kidney disease progression & the impact of these factors relate to timing of referral.
- Identify referral criteria & the impact of early referral a patient's evaluation & listing.
- Describe elements of a transplant evaluation & how these impact candidacy.
