This PI activity requires the review of chart data from a set of your patients with HCV who are receiving treatment.  Select 10 patient charts that satisfy the criteria and then answer the following 2 questions for each chart to confirm that each meets the criteria for inclusion in the practice improvement process used in this activity. If the chart is eligible, you will then answer questions about your diagnosis and management of the patient.
