Duke Friends of Nursing: Nursing Grand Rounds - Medication Safety [Polypharmacy & High Risk Medications with Vulnerable Populations]

Nursing Grand Rounds provides a forum for nurses to share clinical expertise and experiences, evidence-based practices and other topics of interest or need to help improve patient outcomes and enhance the patient experience. The forum, be it live or virtual, is held as planned and provides web accessibility to all interdisciplinary staff to attend from Duke University Health System.

Learning Objectives

Enhance nurses’ knowledge of how high-risk medication (HRM) use for adults ages 65 and older can result in preventable patient harm, and how a quality-improvement project significantly reduced the HRM prescription rate to exceed the national benchmark goal.
