Falls in Older Adults: Assessments and Interventions as a Team Approach

The Duke Population  Health Management Office (PHMO) supports Duke Connected Care (DCC), the physician led clinically-integrated network (CIN), with "results-driven care management, evidence-based practice support, and actionable data analytics" to improve health outcomes. The goal of PHMO Health Optimization Program for Elders (HOPE) in Primary Care (PC) program is to bring focus to the medical complexities of the geriatric population. HOPE in PC offers staff education and professional development activities, such as Teambuilding and Quality Improvement workshops, these practices. This webinar, in partnership with Kerr Tar Area Agency on Aging, focuses on falls in older adults. We provide statistics about the scope of the problem, provide guidance on assessment tools, and interventions, within primary care, but also within the health system and community.
