IBHC Sustainment Training: December 2014

A growing body of research shows that health care providers’ use of existing evidence-based clinical practice interventions is often delayed, ineffectively applied, or not used at all due to a myriad of barriers within the health care system. Providers, patients and overall system barriers often unintentionally impede the provision of demonstrated, evidence-based interventions that are known to positively impact patient outcomes. Nowhere is this more evident than in multi-disciplinary primary care settings, including patient-centered medical homes currently implemented across the DoD. Fortunately, research continues to support the notion that incorporating evidence-based practice into formal clinical pathways of health care delivery among multi-disciplinary team members improves the delivery of service and health outcomes over standard care. This webinar will highlight the key components necessary for effective clinical pathway implementation and will demonstrate how Internal Behavioral Health Consultants (IBHCs) can maximize their role in successful multidisciplinary care. Further, this presentation will provide an overview of the eight clinical pathways developed exclusively by the Services to specifically engage the IBHC as an active and integral part of the multidisciplinary primary care team. 

LEARNING OBJECTIVES:

• Compare and contrast the differences between a clinical practice guideline and a clinical pathway
• Generate the components necessary to effectively implement a clinical pathway for primary care implementation
• Analyze the  important core competencies required among providers for the implementation of a successful multi-disciplinary health care delivery pathway in primary care
