Malnutrition in Older Adults, Post-Acute and Long Term Care, Provider
Frail & elderly patients have complex needs which requires the healthcare team to develop competencies to properly care for them. Some of Duke University’s most respected clinicians and researchers developed an extensive program that identifies risks and needs of frail and elderly patients. These role based and care specific modules provide knowledge and best practices for caring for caring for these medically fragile patients and their families. This program is specifically designed for Post-Acute care focuses on critical areas of care such as delirium, transitions of care, medication management, malnutrition, and shared decision making.
