Nursing Grand Rounds: Population Health and the Social Determinants of Health: Implications for Nursing

To inform nurses regarding the concept of population health and its impact on care provision.  As called out by the National Academy of Medicine, population health, and the influencing social determinants of health, are impacting health and health care like never before.  It is imperative that nurses understand the concepts and are prepared to  assess, plan and intervene as appropriate to improve health care outcomes.
