Objectives:

¤Provide an overview of the impact of heart failure on the US healthcare system in the hospital and SNFs.
¤Summarize the 2016 ACC/AHA/HFSA guideline updates on new pharmacological therapies for HF.
¤Evaluate the mechanisms of action, pharmacologic features, and dosing of existing and new agents for treating and managing HF.
¤Describe strategies that may reduce readmissions, such as adherence to HF guidelines, education, appropriate discharge instructions, and models of care for SNF patients.
