Title: Discharge Medication Errors
 
Objectives
1. Review the medication discharge process and how aspects of this process may contribute to medication errors.
2. Discuss factors in the healthcare system which influence Discharge Medication Errors, including patient factors which may be contributory versus protective against such errors.
3. Explore potential solutions to help improve medication errors and the transition from inpatient hospitalization to outpatient care.
